The hospital scenes were shot at a former Carewest hospital at 1011 Centre Ave NE, Calgary, Alberta, Canada. Carewest was also the location for the Bemidji Police Department in the first 3 seasons of the Fargo TV series. It has since been demolished.
ALIVE. has had three different music score's for its trailers - The Orange County CA band KillSET did the score for "The Doctor Is In" trailer used on the festival circuit. Harry Manfredini who scored several of the Friday the 13th films scored the theatrical trailer. CLOWN aka Michael Shawn Crahan of the band Slipknot did a special score for the promotion of ALIVE.'s early premier on Knotfest.
